MEDICAL CONSULTATION

One-time consultations are available for individuals with complex health conditions or specific wellness or performance goals. These services are non-member, advisory in nature, and do not establish an ongoing physician-patient or DPC relationship unless specifically arranged.

Consultation fees do not include labs, imaging, medications, supplements, or therapies unless specified. Determination of complexity and appropriateness is at the provider’s discretion.

This consultation is intended for individuals seeking a comprehensive medical overview, second opinion, or personalized wellness or performance guidance. These visits are typically one-time consultations.

Fee: $500 — Up to 60 minutes

If, following the consultation, both the provider and patient determine that DPC membership is appropriate and clinic capacity allows, the $300 enrollment fee will be waived.

This consultation is intended for individuals with complex or multi-system medical concerns requiring focused evaluation and care planning. Examples may include cancer support, Lyme disease, autoimmune disorders, or other chronic or inflammatory conditions.

Fees:  

$350 — Up to 60 minutes (Initial Consultation)
$250 — Per follow-up visit

Follow-up frequency is determined based on clinical needs and provider discretion.

An EBOO Medical Screening is required prior to initiating EBOO therapy.

Visit includes:

·  Evaluation by Dr. Tomas Gigena, MD

·  Determination of appropriateness for EBOO therapy

·  Ordering of required laboratory testing, including G6PD screening

Fee: $100 — Screening visit for Non-Members

G6PD laboratory testing is included in Screen Visit Fee. Any additional laboratory testing is billed separately. Completion of screening does not guarantee approval for EBOO therapy, as participation is subject to medical clearance and provider discretion.

A Medical Screening is required prior to initiating Shockwave Therapy.
