ONLINE ENROLLMENT FOR NEW DIRECT PRIMARY CARE (DPC) PATIENTS
(IV Therapy only or wellness therapies, please call the center directly).
MEMBERSHIP:
Membership is month by month with 4 auto-charge options
Monthly, 3 month, 6 month, full-year (discount)
* The first three months are required at time of enrollment.
ENROLLMENT FEE:
There is a one-time enrollment fee of $300 charged for new memberships at time of enrollment for onboarding, account set-up, intake process, health history, and program placement. Enrollment fee is charged per adult 18+.
MONTHLY MEMBERSHIP PRICING:
$80 Individual (19-100+)
$40 Young Adult (18-21 with paying adult and living with parent member)
$20 Children ( under 18 with adult membership)
$200 Family (2 parents + 2 children w/ unlimited same family children under 18)
$880 Full year Individual discount when PAID IN FULL
(*membership includes most primary care services. It also includes discounts on labs, imaging, prescriptions, supplements or the integrative wellness therapies).
PAYMENT:
A credit card is required on file for monthly auto-charge and other charges.
Cash pay only accepted for 3 month, 6 month or 1 year membership
Payment or arranged payment plan is due at time of service.
You will not be charged until you are accepted as a patient.
CANCELLATION:
A cancellation request must be submitted in writing.
A 30-day notice is required with 3 months minimum membership.

NEW MEMBER ESTABLISHING CARE:
Once you register, you will get a welcome email. The next step is to establish care
by scheduling your first appointment, touring the clinic, and meeting our staff. Please bring your most recent labs, health records or health timelines, your prescriptions or supplements and your questions/ health goals.
Enrollment for Dr. Tomas Gigena, MD is by invite only. His is currently operating from a waitlist. Stephanie Tullar, FNP-BC is accepting new patients. Our amazing staff will be more than happy to help you navigate the process and with any questions.
